Facility Services

Skilled Nursing: Skilled nursing services generally are designed to follow a hospital stay, either on a short- or long-term basis. Professional nursing care, provided by dedicated teams, focuses on the special needs and concerns of each individual resident. Facility teams provide nursing care through a variety of innovative, comprehensive and interdisciplinary programs, and by involving the physical, occupational and speech therapists.

Rehabilitation: Whether you are recovering from hip replacement surgery, a stroke, a recent fall, or any other health condition requiring rehabilitation, facility teams offer comprehensive programming designed to meet your needs. Your rehabilitation begins with a clear understanding of your goals and a commitment to return you to the highest possible level of function and independence. Facilities endeavor to employ exceptional physical, occupational and speech-language therapists to deal directly with all areas of functional loss. Facility clinicians are licensed professionals with the education, experience and advanced expertise required to achieve successful rehabilitation outcomes.

Assisted Living: An assisted living facility is designed especially for residents needing less intensive service in a safe residential environment. Facility teams provide supervision or assistance with the activities of daily living, coordination of healthcare services, individualized supportive services, as well as monitoring of resident activities to help ensure health, safety and well-being. Staff is encouraged to uphold a philosophy of service that focuses on maximizing each resident’s independence and dignity in an assisted living facility.

Alzheimer’s / Dementia Care: Care is provided in safe environments focused on the special needs of individuals with cognitive disorders. Residents enjoy appropriate activities and programs supervised by staff that has been trained in the care of those who are cognitively impaired. This focused care enhances safety and enjoyment of the residents and provides peace of mind to family members.

Adult Day Care: Adult day care services enable a facility to extend its model of care to adults in the community who, because of mental or physical impairment, are not able to be left alone during the day. It is a place of socialization where the effects of isolation can be prevented. Spending the day in adult day care means spending the day among new friends and a caring staff in a safe environment. Participants are offered supervised activities, peer support, companionship, recreation, and some assistance with the activities of daily living.

All facilities are licensed by the individual licensing authority within the state in which they operate. Each facility is a member of their individual state’s professional association, and a member of the American Healthcare Association, the national professional organization for licensed Nursing Facilities. All services provided by a Grace Healthcare supported facility are offered without regard to race, color, national origin, disability, age, religion, sex, citizenship status, pregnancy, veteran status, or any other status protected under federal, state, or local law.
